Holds can be placed for a minimum of 1 month and a maximum of 4 months (can be extended in the case of pregnancy or medical necessity only). There is a one-time $10 administrative hold fee (the fee is waived for prenatal clients). Maximum of two holds per year. You must submit this form by the 15th of the current month to go on hold for the following month.
